Action, progress & evidence journal
Record what you did, what you observed, what you learned, factors outside your control and your next step — facts kept separate from interpretation.
Steps
- Log the action you took.
- Record what you observed and how you felt (optional).
- Note lessons and outside factors.
- Choose the next step; review weekly or monthly.
Background
Structured reflection (for example Gibbs' reflective cycle, 1988) separates description from evaluation. The journal keeps observable facts apart from personal or symbolic meaning, and never treats coincidences as proof.
Limitations
- Summaries count your entries; they never infer that a practice caused an outcome.
